First Communion and Confirmation Registration Forms

Registration in the Parish Office

Sunday, September 28th to Saturday, October 11th from 9 a.m. to 4 p.m.

Requirements:

Registration form (The 2026 Registration Forms can be found on our website).
- Child’s birth certificate
- Child’s baptism certificate

Spaces are limited
Enrollment will be first-come, first-served

To register your child for the church sacraments, please download the the registration form that applies to your child and fill it out carefully.
