Copy of candidate's Baptism Certificate
Sponsor Information (If you have already chosen one)
- Copy of Confirmation certificate, if single
- Copy of church marriage certificate, if married

To register your child for the church sacraments, please download the the registration form that applies to your child and fill it out carefully.
